Medical Consent Forms for Parents

If your child has a long term condition that you feel we should be aware of please complete this form below. This form is for conditions such as asthma, allergies, or any on-going condition you feel we as a school should be aware of.

Long term condition form

If your child is unwell but is well enough to return to school, but requires the administration of medication on site, please compete this form below. For example if your child has a cough, cold etc.

Administration of Medication form

Please be aware that any child that experience sickness and/or diarrhoea MUST stay home for a minimum of 48 hours after experiencing the symptoms.
